Below are a series of recordings walking you through the process of creating a Google Form. After working through this part of the workshop, you will have created a more Google Form that you could use in your classroom and shared that Form here on the wiki. While not mandatory, you may want to actually use the Google Form created today with your students. If you do, please share your results where indicated in the optional section below.

Activity: Embedded here is a Google Form that you are to complete. Please add at least 2 uses for Google Forms in your classroom. I have also embedded the response spreadsheet so that you can view other's posts. Be sure to read previous responses so as not to repeat what others have previously said.

Hopefully the brainstorming activity has given you some great uses for Google Forms. Now it's time for you to create your own form.
Directions: Go to http://docs.google.com and sign in to your Google account if you are not already. Then, start a new form. Your form should include the following:
1. A title
2. Directions for your students
3. 5 questions, other than those where students will type their name, date, and/or class.
4. Please use at least two different types of questions in your form.
5. Choose a theme other than the default one.
The goal here is to create a form that you might actually use in class or as a take home assignment for your students. It is suggested that you choose a current or future unit for Google Form.
4. Activity: Sharing and Critiquing Google Forms
Part 1: Posting your Google Form
Directions: In the discussion area for this page, start a new topic. In your post, include the following:
1. A link to your form
2. A summary of the assignment or class activity the form will be a part of.
3. A short explanation of why you chose the question types you used for the form.
Directions: Below is embedded your Google form posts to the discussion board. You will comment on two of your colleagues forms by responding to their posts. In your critique, comment on the following:
1. What did you like/dislike about the form?
2. What are the strengths of the form?
3. What are areas that you see for improvement in the form?
4. Overall, is this a successful application of Google Forms? Please explain.
Once you leave today and get back to your classrooms, you are encouraged to actually assign your students the form you created today. If you do, please share how it went in the discussion are on this page.
